Deposit questionnaire Punk Hollow Labrador Retriever

To reserve a puppy, a deposit is normally required.  Our puppies are adopted very quickly.  Most litters have deposits prior to being born.  

If interest in reserving a puppy there is several steps that need to be taken.  Please follow the directions below

1. Fill out a  puppy application.
2. We then need to speak on the phone.
3. If we are both satisfied with one another, a deposit questionnaire, will need to be filled out located below.
4. The agreed price NEEDS to be filled out.  The price needs to  reflect the agreed price, including any agreed discount.  The agreed price is final. 
5. After the questionnaire is filled out a deposit can be made.
6. Deposits can be *US POSTAL MONEY ORDER, Bank Money order, Personal Check,  Checks and Money orders will be made out to Nicholas Portentoso, and mailed to 9910 W. Township Rd. 132, Fostoria, Ohio.
7. DEPOSITS ARE POSITIONED UPON RECEIPT.  WE RECOMMEND SENDING THE DEPOSIT VIA PRIORITY MAIL WITH A TRACKING NUMBER.  THEN TEXT THE NUMBER TO ME  419 939-3192) AND YOUR POSITION WILL BE LOCKED IN IMMEDIATELY UPON RECEIPT OF THE TRACKING #.
8. The deposit are subtracted from the final cost of the puppy.  Final payment is upon picking up the puppy.  NO PERSONAL CHECKS will be accepted.  ONLY CASH, US POSTAL MONEY ORDER, BANK CASHIER CHECKS. 
9. Deposits are non-refundable under normal circumstances.
10. Deposits are non-transferable.
